Each LB Poly athlete is required by CIF rules to complete certain documents for play in a CIF section high school. In addition to the required CIF forms there are also required LB Poly Football documents that are included with your packet.

It is very important that you athlete and parent fill-out these forms for play and eligiblity in sports. Theses documents MUST be turned in to the proper personell at LB Poly (See documents for more information). If your student does not hand in their packet, they will not be eligible for play in ANY Poly sport.
